Brain Iron Levels Linked to Cognitive Decline in Alzheimer’s - Technology Networks

Researchers using MRI have found that iron accumulation in the outer layer of the brain is associated with cognitive deterioration in people with Alzheimer's disease, according to a study published in the journal Radiology.

Alzheimer's disease is a progressive type of dementia that impairs and eventually destroys memory and other brain functions. There is no cure, although some treatments are thought to slow the progression.

Previous research has linked abnormally high levels of iron in the brain with Alzheimer's disease. Iron deposition correlates with amyloid beta, a protein that clumps together in the brains of people with Alzheimer's disease to form plaques that collect between neurons and disrupt cell function. Associations have also been found between iron and neurofibrillary tangles, abnormal accumulations of a protein called tau that collect inside neurons. These tangles block the neuron's transport system, which harms the communication between neurons.

It is known that deep gray matter structures of patients with Alzheimer's disease contain higher brain iron concentrations. Less is known about the neocortex, the deeply grooved outer layer of the brain that is involved with language, conscious thought and other important functions. The neocortex is challenging to assess by MRI, as the anatomy of the area makes MRI prone to distortions, signal decays and artifacts.

"The best solution to minimize these artifacts would be using ultra-high-resolution scans," said study coauthor Reinhold Schmidt, M.D., professor of neurology and chairman of the Department of Neurology at the Medical University of Graz in Graz, Austria. "However, in the clinical setting, scan time is a limiting factor, and a compromise has to be found."

Qure.ai gets FDA clearance for its head CT scan product qER - MobiHealthNews

Mumbai-based healthcare AI startup Qure.ai earlier this week announced its first US FDA 510(k) clearance for its head CT scan product qER. The US Food and Drug Administration’s decision covers four critical abnormalities identified by Qure.ai’s emergency room product.

The AI tool can now be used to triage radiology scans with intracranial bleeds, mass effect, midline shift, and cranial fractures. Qure.ai claims that two of these capabilities – cranial fractures and midline shift are exclusive to qER. This means that the newly cleared qER suite will be able to triage nearly all critical abnormalities visible on routine head CT scans.

July 2, 2020 -- Dr. László Tabár, a radiologist and oncologist from Falun, Sweden, is known worldwide as the "father" of mammography screening. He is certainly one of the key people behind the current national screening program for breast cancer in Sweden, but his research has also formed the basis for the introduction of screening programs in many other countries.

In this interview, Tabár shares the fascinating story of why he has dedicated the past 50 years of his life to introducing and improving mammography screening, and thereby reducing mortality in breast cancer -- a journey colored by groundbreaking research results and success but also by opposition and a struggle to convince authorities of the benefits.

He gives us the story behind the introduction of mammography screening in Sweden and the intriguing results shown in two of his latest studies, published in 2018 and 2020, proving the benefits of screening independent of treatment regime -- something he calls "one of the greatest accomplishments in clinical cancer research during the past 50 years."

A key person in the history of mammography screening was Prof. Barbro Westerholm, chief of the National Board of Health and Welfare, who played a very important role in recommending nationwide screening after our group published its seminal trial in the Lancet. She is still a member of the Swedish Parliament and fighting for mammography screening -- nowadays for older women.

July 2, 2020 -- Children infected with the SARS-CoV-2 virus may develop asymptomatic or mildly symptomatic COVID-19 but later experience a second, severe inflammatory illness in the brain that shows up as signal changes in the splenium of the corpus callosum on MRI, according to a study published July 1 in JAMA Neurology.

The findings suggest that clinicians caring for children infected with SARS-CoV-2 should be on the alert for secondary neurological symptoms, wrote a team led by Dr. Omar Abdel-Mannan of Great Ormond Street Hospital for Children in London, U.K.

"Increasing reports of children developing systemic inflammatory response requiring intensive care (labeled pediatric multisystem inflammatory syndrome temporally associated with COVID-19) and a further group of children with far less severe, Kawasaki-like disease ... suggest that despite the typically mild acute infection, children may be at high risk of a secondary inflammatory syndrome," the group wrote.

Since the COVID-19 pandemic hit, some researchers have reported on neurological manifestations of COVID-19 in adults, but less is known about these manifestations in children, the group noted. To address this knowledge gap, Abdel-Mannan and colleagues assessed 50 patients under the age of 18 who presented at the hospital between March 1 and May 8 with the SARS-CoV-2 virus; infection was confirmed by either reverse transcription polymerase chain reaction (RT-PCR) assay or IgG antibody test.

Of these 50, 27 had symptoms suggesting COVID-19 pediatric multisystem inflammatory syndrome in children (MIS-C). Four of these 27 (14.8%) had neurological symptoms such as encephalopathy, headaches, brainstem and cerebellar signs, muscle weakness, and reduced reflexes. All four of these children required admission into the intensive care unit, and all four tested negative for other possible causes of infection.

July 2, 2020 -- Autonomous artificial intelligence (AI) algorithms aren't close to being safe enough to replace radiologists, and the U.S. Food and Drug Administration (FDA) should hold off for now on developing regulatory pathways for autonomous AI in radiology, according to the American College of Radiology (ACR) and the RSNA.

Autonomous algorithms would include AI models that identify normal radiological examinations or rule out critical diseases, according to the organizations. To date, there's a lack of research-based criteria for ensuring that these types of algorithms are generalizable across clinical sites, and there's also a considerable body of published research that suggests they often perform poorly across heterogenous patient populations, according to ACR Chair Dr. Howard Fleishon and RSNA Chair Dr. Bruce Haffty.

Fleishon and Haffty wrote to the FDA in response to a February FDA workshop on the evolving role of AI in radiological imaging.

"In light of the broad heterogeneity in imaging equipment and image acquisition protocols and the lack of a definable mechanism to ensure the longitudinal performance of the algorithm, we are concerned that autonomously functioning algorithms of the varieties discussed at the workshop would pose a significant risk to patient safety," they wrote.

If the goal is to remove physicians from image interpretation, then the public must be assured that AI algorithms will be as safe and effective as the physicians they replace, according to Fleishon and Haffty. That also includes the ability to incorporate available context and identify secondary findings.

July 2, 2020 -- Perimeter Medical Imaging AI has completed its plan to go public on Canada's TSX Venture Exchange via a reverse takeover and private placement.

Perimeter took over New World Resource in the reverse takeover, and at the same time closed a private placement worth $7.4 million ($10 million Canadian). Shareholders of New World stock received shares of Perimeter as part of the deal. The company's stock is trading under the symbol PINK.

Perimeter has developed OTIS (optical tissue imaging system), which aims to reduce the reoperation rate for breast cancer patients by acquiring high-resolution subsurface image volumes across the surface of removed tissue that can be used during breast cancer surgery. OTIS allows surgeons to assess whether entire tumors were removed successfully.

The company also soon will release a SonoSite PX point-of-care ultrasound system that can be integrated into more COVID-19 care pathways.

The Fujifilm SonoSite COVID-19 Point-of-Care Ultrasound Acceleration Research Grant, which was created to support the Emergency Medicine Foundation COVID-19 research grant program, will accept applications through August 21. Winners will be announced in September 2020.

July 1, 2020 -- The American Telemedicine Association (ATA) has sent a letter to U.S. lawmakers, requesting they advance permanent telehealth reform.

U.S. Congress expanded access to telehealth services during the COVID-19 pandemic by giving the Department of Health and Human Services (HHS) and the Centers for Medicare and Medicaid Services (CMS) the authority to waive restrictions on Medicare telehealth services.

The temporary change resulted in many people using telemedicine for the first time and shows more may be more inclined to use it in the future, according to the ATA. With these positive results, the ATA has asked Congress to focus reform on numerous priorities, including the following:

Allow the HHS and the CMS to maintain the authority to add or remove eligible telehealth services.Remove the geographic and originating site restrictions to ensure that all patients can access care at home and other appropriate locations.Ensure federally qualified health centers and rural health clinics can continue to offer telehealth after the pandemic.

Many companies and organizations, including the Johns Hopkins Health System, the Michael J. Fox Foundation for Parkinson's Research, and the American College of Physicians, signed the letter.

June 30, 2020 -- An artificial intelligence (AI) algorithm can highlight incidental cases of pulmonary embolism (PE) on contrast-enhanced chest CT exams that were performed for reasons other than to detect PE, according to a presentation at the recent online annual conference of the Society for Imaging Informatics in Medicine (SIIM).

A team of researchers from Yale University retrospectively evaluated a machine-learning algorithm on chest CT exams performed at their institution over a two-month period. They found the model to be extremely sensitive and specific.

"This algorithm could help the radiologist detect incidental PEs, it could expedite reporting of cases with incidental PEs, and can help identify patients that may need immediate medical attention," said first author and presenter Dr. Anna Bader.

To assess its performance, the researchers retrospectively applied the model to 2,632 consecutive contrast-enhanced chest CT studies from January 1, 2018, to February 28, 2018. CT pulmonary angiograms were not included.

Lantheus to discuss PyL imaging agent at SNMMI 2020

July 1, 2020 -- Lantheus Medical Imaging will showcase the potential of its PyL (F-18 DCFPyL) radiopharmaceutical at the virtual Society of Nuclear Medicine and Molecular Imaging (SNMMI) 2020 annual meeting, July 11-14.

PyL is an investigational prostate-specific membrane antigen-targeted small-molecule PET radiopharmaceutical that aims to visualize prostate cancer. Data from company-sponsored studies of PyL will be presented on July 11 in the Science Pavilion at SNMMI 2020.

The information that will be presented evaluates the diagnostic performance and clinical impact of PyL in patients with biochemical recurrence of prostate cancer. Another abstract focuses on a digital solution Latheus is developing with PyL in the hopes of supporting prostate cancer staging with the use of the automated miPSMA index of the PET/CT PyL-prostate specific membrane antigen (PSMA) images.

July 1, 2020 -- Artificial intelligence (AI) software developer Elucid Bio is highlighting a study showing that its vascuCAP AI algorithm was about 70% more accurate in predicting strokes than stenosis-based methods over a six-year time period.

Researchers from the University of Maryland's Center for Vascular Diagnostics and the U.S. National Institute of Health Vascular Imaging Core Facility sought to improve the prediction of major adverse neurological events, including stroke, transient ischemic attacks, and amaurosis fugax by applying Elucid Bio vascuCAP software to traditional carotid imaging.

On the right, Elucid Bio's vascuCAP software transforms the scan into a 3D visualization and quantitation of plaque tissue composition and structural measurements. vascuCAP was validated in the CRISP trial to inform highly accurate prediction of major adverse neurological events such as stroke. The findings indicate that vascuCAP's algorithms for risk stratification in carotid atherosclerosis are better predictors of stroke than stenosis assessment alone, according to the company. The technology, which is in clinical use, could improve risk assessment for stroke, including patients diagnosed with COVID-19, according to the findings.

The interim results will be presented at the Society for Vascular Surgery Online Summer 2020 conference on July 2.

JFL Board DIscusses Rebuilding the Hospital - St. John Source

With the deadline for bidding on the rebuilding of the Gov. Juan F. Luis Hospital less than a month away, board members were eager Thursday night to get started sorting through the applications to find a perfect fit and had to be reminded that they will need expert help in reaching a decision.

Since there are only three members of the Gov. Juan Luis Hospital and a minimum of five is necessary for a quorum to conduct business. Dr. Anne Treasure, the board chairwoman, said Thursday night’s meeting was an “information session.” Treasurer Christopher Finch and Faye John-Baptiste were joined by senior hospital administrators to discuss the upcoming rebuild of the facility by FEMA after the 2017 hurricanes and other items.

The request for proposals to rebuild the facility has drawn bids by 67 contractors, so far. With a July 17 deadline, more companies may bid for the construction project. But while the board members expressed excitement about sorting through the prospects to find the perfect fit, interim Chief Executive Officer Dyma Williams and others reminded them they need professional engineers, architects, lawyers and others to help make the final decision.

“I would like to see a way to develop a quorum and a board with an attorney, an accountant, an architect and someone with physical plant experience,” Treasure said.

Finch said that the Division of Property and Procurement will probably lead the rebuilding project.

June 26, 2020 -- An artificial intelligence (AI) neural network used with lung CT scans can help clinicians assess cancer severity and develop treatment options, according to research conducted by a team from Stanford University.

A group led by Olivier Gevaert, PhD, created a neural network called LungNet to gather lung cancer information from CT scans, particularly those from adults with non-small cell lung cancer (NSCLC), which represents the majority of diagnoses of the disease.

The group trained the network on four patient cohorts with NSCLC from four medical centers. LungNet accurately predicted overall survival in all four patient groups, accurately categorized benign and malignant nodules, and it further classified nodules according to cancer progression, according to the researchers.

"LungNet demonstrates the benefits of designing and training machine learning tools directly on medical images from patients," contributing author Qi Duan, PhD, said in a statement released on June 24 by the National Institute of Biomedical Imaging and Bioengineering (NIBIB), which funded the research. "This is an outstanding example of how machine-learning technology can be a cost-effective approach to advance disease detection, diagnosis, and treatment."

June 26, 2020 -- The future is now for 3D imaging technologies such as cinematic rendering, 3D printing, simulation training, virtual reality, and augmented reality, according to a June 26 presentation at the virtual annual meeting of the Society for Imaging Informatics in Medicine (SIIM).

"These are really exciting times to look into 3D technologies because of custom and personalized medicine, as well as value-based care," said Dr. Ramin Javan of George Washington University (GWU) Hospital in Washington, DC.

Advancements in 3D technologies have facilitated personalized and targeted medical care, better patient understanding and satisfaction, and multispecialty and multi-institutional collaboration, Javan said.

"As gateways of imaging, [radiology departments] can help consolidate these 3D techniques in hospitals and academic institutions," he said.

June 26, 2020 -- Philips Healthcare has reported a low-level security issue with its ultrasound scanners that could enable unauthorized personnel to bypass authentication and log in to systems.

In a security alert, the company said that the issue only applies to local access to affected scanners, and it requires a "high skill level" to exploit. If an exploitation does occur, the unauthorized user might be able to enable and access features on the ultrasound scanner that were not included with the purchase of the system.

The company noted that it believes that the vulnerability is not a device safety issue, and it does not believe the security problem is a safety hazard to patients. Philips noted that it hasn't received any reports of the vulnerability being exploited in clinical use.

The company said the vulnerability applies to Ultrasound ClearVue versions 3.2 and prior, Ultrasound CX versions 5.0.2 and prior, Ultrasound Epiq and Affiniti versions VM5.0 and prior, Ultrasound Sparq version 3.0.2 and prior, and Ultrasound Xperius.

Philips said it has released or is planning to release new software updates for the scanners that address the vulnerabilities.

A new case series published in the journal Radiology examines the spectrum of imaging findings in children with the post-COVID-19 inflammatory condition known in the U.S. as Multisystem Inflammatory Syndrome in Children (MIS-C).

The array of findings includes airway inflammation and rapid development of pulmonary edema, coronary artery aneurysms, and extensive intra-abdominal inflammatory changes.

In April 2020, Evelina London Children's Hospital in London, U.K., experienced a surge of children with a multi-system hyperinflammatory syndrome. The children had a variety of symptoms, including fever, headaches, abdominal pain, rash and conjunctivitis. Clinical features and lab findings shared some similarities to those of Kawasaki disease--which causes inflammation in the walls of blood vessels--Kawasaki-disease shock syndrome or toxic-shock syndrome, although atypical and more severe.

ACR Announces COVID-19 Imaging Research Registry - Imaging Technology News

June 25, 2020 — The American College of Radiology (ACR) Center for Research and Innovation (CRI) is pleased to announce the development of the COVID-19 Imaging Research Registry (CIRR), an effort by the ACR CRI and the ACR Data Science Institute in collaboration with the ACR and the Society of Thoracic Radiology (STR). Sharyn Katz, M.D., director of research for thoracic radiology at the University of Pennsylvania, chairs the effort’s multiple-disciplinary steering committee, which includes representation from across the imaging community as well as medical specialties focused on critical care and pulmonary medicine.

This registry is a primary step to advance the understanding, prevention and treatment of COVID-19. It is a cross-disciplinary initiative with collaborations extending beyond radiology to engage other clinical experts and diagnostic modalities, including clinical medicine, genetics, biomarker discovery, laboratory sciences and others. Participating sites will contribute demographic information, clinical data on signs and symptoms, imaging exams, laboratory test data and outcomes for U.S. patients tested for COVID-19. The registry is being constructed to allow for linkage of information to other existing COVID-19 registries and datasets, with use and development of common data elements.

“This registry will support further COVID-19 research and data. We are thrilled to work with our participating sites to empower our members and their radiology practices and help improve the quality of care for these patients,” said Etta D. Pisano, M.D., FACR, chief research officer of the ACR.

“The data registry will integrate imaging and clinical data, thus enabling investigators to address crucial questions related to the diagnosis, treatment and management of patients with COVID-19 through collaborative efforts. The ultimate goal is to aid patients affected by this emerging disease,” said Jane P. Ko, M.D., president of the STR.

The ACR CRI is prepared to leverage its extensive history in clinical research trials and observational studies, taking the next evolutionary step toward creating sustainable research data registries. The CIRR will provide fertile opportunity for discovery of new information about the clinical presentation and course of the virus in diverse populations across the U.S. In addition, it will serve as a resource for the development and testing of artificial intelligence and machine learning algorithms to be used to predict the course and severity of the disease in given populations and subpopulations and to develop early diagnosis models and potentially even targeted treatments.

RSNA Launches International COVID-19 Open Radiology Database - Imaging Technology News

June 25, 2020 — The Radiological Society of North America (RSNA) and the RSNA COVID-19 AI Task Force today announced the launch of the RSNA International COVID-19 Open Radiology Database (RICORD). RICORD is envisioned as the largest open database of anonymized COVID-19 medical images in the world. More than 200 institutions around the world have expressed their interest in participating.

The database will include supporting clinical information and expert annotations. It will be freely available to the global research and education communities.

“The strong positive reaction speaks to the determination of the global radiology community to contribute its resources and expertise to addressing the pandemic,” said RSNA COVID-19 AI Task Force chair Matthew P. Lungren, M.D., M.P.H., assistant professor of radiology at Stanford University and associate director of the Stanford Center for Artificial Intelligence in Medicine and Imaging. “This effort is the result of countless hours by volunteer task force members and a broad community of radiologist annotators led by our close partners at the Society of Thoracic Radiology.”

Shortly after the first news of the pandemic, scientists isolated the virus and quickly sequenced the genome. The sequenced genome was immediately made available to the entire worldwide research community and became the inspiration for RICORD.

“This unprecedented spirit of collaboration accelerated clinical testing, therapeutic drug discovery, epidemiologic tracking and vaccine development,” Lungren said. “All of these advancements were completed in weeks to months, rather than the typical pace of months to years, and were catalyzed by this act of open-source scientific collaboration. This is perhaps one of the most dramatic examples of an open source research effort saving countless lives.”

As it is known from the international experience with COVID-19 diagnostics, the PCR test has quite limited accuracy and a long processing time. Radiology diagnostics became the frontline area in the fight with the pandemic: sensitivity of computed tomography is around 98%.

The Moscow Center for Diagnostics and Telemedicine (CDT), under the leadership of Prof. Morozov, Chief Regional Radiology Officer of Moscow, successfully developed common operation principles of radiology departments that applied to all medical facilities, including public, private, and federal, and would allow to diagnose COVID-19 efficiently and promptly.

The infrastructure of Moscow radiology diagnostic services was prepared in advance in 2012-2018. IT provision was arranged, and city clinics were provided with CT scanners. Medical facilities connected about 1000 devices to the Unified Radiological Information Service (URIS). This service conducts a cycle of diagnostic study in a single digital space. Radiologists' work was organized remotely.
